Low Testosterone Levels in Men: What You Need to Know

Testosterone deficiency in men has become an increasingly important topic in medical research, as more connections between low levels of this hormone and various health conditions are being discovered. Diabetes, metabolic syndrome, and obesity are just some of the issues associated with low testosterone. While low testosterone is not a direct cause of these conditions, the association is significant enough to warrant attention.


Does Low Testosterone Mean Poor Health?

Recent studies suggest that there is a connection between low testosterone levels and other medical conditions. In one study conducted on 2,100 men over the age of 45, it was found that the chances of having low testosterone were:

  • 2.4 times higher in obese men
  • 2.1 times higher in men with diabetes
  • 1.8 times higher in men with high blood pressure


Experts do not claim that low testosterone directly causes these conditions. Instead, men with health problems or poor overall health often have lower testosterone levels.


Diabetes and Low Testosterone

The link between diabetes and low testosterone is well documented. Men with diabetes are more likely to have low testosterone levels, and those with low testosterone are more likely to develop diabetes. Testosterone helps tissues absorb sugar from the blood in response to insulin. Men with low testosterone often have insulin resistance, meaning their bodies must produce more insulin to maintain normal blood sugar levels.


Obesity and Low Testosterone

Obesity and low testosterone are closely related. Fat cells convert testosterone into estrogen, which lowers testosterone levels. Additionally, obesity reduces the level of SHBG (sex hormone-binding globulin), a protein that transports testosterone in the blood. Less SHBG means less free testosterone. Exercise and weight loss can significantly increase testosterone levels in obese men.


Metabolic Syndrome and Low Testosterone

Metabolic syndrome, which includes abnormal cholesterol levels, high blood pressure, abdominal obesity, and high blood sugar levels, is also associated with low testosterone levels. Short-term studies have shown that testosterone replacement can improve blood sugar levels and reduce obesity in men with low testosterone. However, testosterone therapy may increase the risk of heart disease and is therefore not used to treat metabolic syndrome.


Testosterone and Heart Disease

Testosterone has different effects on the arteries. Some experts believe that high testosterone levels may contribute to higher rates of heart disease and high blood pressure in younger men. On the other hand, low testosterone is associated with insulin resistance, obesity, and diabetes, all of which increase cardiovascular risk. Men with diabetes and low testosterone are more likely to suffer from atherosclerosis, or thickening of the arteries.


Low Testosterone and Fertility

Low testosterone levels can significantly affect fertility in men. Testosterone plays a key role in sperm production, and its deficiency can lead to reduced spermatogenesis. This means that men with low testosterone levels may experience:

  • Reduced sperm production: Low testosterone can decrease sperm count and quality.
  • Reduced sexual desire: Low testosterone may lower libido, affecting the frequency of sexual intercourse.
  • Erectile dysfunction: Low testosterone can contribute to erection problems, making it harder to conceive.
  • Hormonal imbalance: Low testosterone may be associated with an imbalance of other hormones, negatively impacting sperm quality.

If fertility problems exist, it is recommended to consult a doctor or fertility specialist to determine the exact cause and find an appropriate solution.


Testosterone and Other Conditions

Low testosterone often occurs alongside other medical conditions:

  • Depression: A study of nearly 4,000 men over the age of 70 showed that those with the lowest testosterone levels were more than twice as likely to suffer from depression.
  • Erectile Dysfunction (ED): Erectile problems and low sexual desire are common symptoms of low testosterone. While most cases of ED in older men are caused by atherosclerosis, checking testosterone levels may be justified.


Testosterone Replacement Therapy

The question that remains is whether low testosterone causes or worsens medical problems like diabetes, or if individuals with these problems are simply more prone to low testosterone. Studies that would answer this question are ongoing, but it will take years before we know the results. In the meantime, testosterone replacement therapy has not been proven to improve any health condition except for testosterone deficiency and its symptoms. The decision to undergo testosterone replacement therapy should be made in consultation with a doctor.


Testosterone replacement therapy (TRT) should only be used in men with low testosterone to treat erectile dysfunction or low libido when other therapies have not been effective. It is also indicated for transgender individuals transitioning from female to male.
